Health for a Better World

In 1991, the Sisters of St. Joseph established Santa Rosa Memorial’s first mobile health clinic with the goal of bringing quality healthcare to underserved communities. Today, we proudly continue their vision of health and wellness for all through a range of programs across Sonoma County. Working in collaboration with dozens of community partners, Providence is a major local player in reducing health disparities and providing access to vulnerable children, seniors, farmworkers, and more. 

Our vision is to eliminate health disparities in Sonoma County by addressing the social determinants of health as well as challenges with access to care. Rooted in our values and centuries-old history as a leader in care for the poor and vulnerable, we convene passionate and experienced philanthropists, medical professionals, volunteers and community partners who are called to address Providence’s key focus areas of mental and behavioral health, access to quality health and dental care, support for aging adults and safe and affordable housing.

Mobile Clinics

Our dedicated and highly trained team provides the safety net and
entry point to medical care for uninsured Sonoma County residents. We provide treatment for minor medical conditions, urgent and episodic care, health screenings and immunizations and chronic disease management for the most vulnerable in our community.

Dental Health

Our fixed-site clinic provides more than 8,000 patient visits per year for low-income, uninsured and under-insured children ages 0-15 years of age, with a targeted effort on children 0-5 years old, patients with special needs, and pregnant women. The clinic provides dental exams, x-rays, cleanings, sealants, fillings, root canals, trauma care, pain management and more. The mobile clinic travels throughout Sonoma County to high-need sites, including elementary schools, providing care to more than 4,500 low-income people who cannot otherwise access dental care. 

Care Network

We promote wellness, disease prevention and independence in self-management of chronic conditions through home or office visits by nurses and social workers offering comprehensive support and care management services. Our team aids in areas such as disease education, financial
resources, home safety concerns, homeless services navigation, and substance use treatment navigation.
