Health for a Better World
In 1991, the Sisters of St. Joseph established Santa Rosa Memorial’s first mobile health clinic with the goal of bringing quality healthcare to underserved communities. Today, we proudly continue their vision of health and wellness for all through a range of programs across Sonoma County. Working in collaboration with dozens of community partners, Providence is a major local player in reducing health disparities and providing access to vulnerable children, seniors, farmworkers, and more.

Your Impact
Mobile Clinics
Our dedicated and highly trained team provides the safety net and entry point to medical care for uninsured Sonoma County residents. We provide treatment for minor medical conditions, urgent and episodic care, health screenings and immunizations, and chronic disease management for the most vulnerable in our community.
Community Dental Clinic (Fixed Site)
The clinic provides dental exams, X-rays, cleanings, sealants, fillings, root canals, trauma care, pain management and more. Averaging more than 8,000 patient visits per year, the clinic cares for low-income, uninsured and under-insured children from birth to 15 years of age (with a targeted effort to reach children under 5 years old), patients with special needs and pregnant women.
Mobile Community Dental Clinic
Offering the first mobile dental clinic in the county, we travel throughout Sonoma County to high-need sites, providing care to more than 4,500 low-income people who cannot otherwise access dental care. Our team provides dental treatment at elementary school sites allowing greater access for children in need. The mobile dental team also provides most of the same basic care as our fixed-site dental clinic.
Mighty Mouth Dental Health Education
The first of its kind and longest running in the county, this school-based prevention and education dental program targets children in preschool through sixth grade throughout Sonoma County. The goal is to improve the oral health of school-aged children through education, responsibility, teaching and reinforcing good habits, and referring children in need of treatment. Our dedicated team visits 29 school sites and provides 9,000 screenings and education sessions per year.
CARE Network
We promote wellness, disease prevention and independence in self-management of chronic conditions through home or office visits by nurses and social workers offering comprehensive support and care management services. Our team aids in areas such as disease education, financial resources, home safety concerns, homeless services navigation and substance use treatment navigation.
Promotores de Salud (Health Promoters)
We partner with this trusted community resource, with its specialized knowledge and experience in bridging language and culture, to provide health education, referrals and access to services for individuals, families and communities.
